- Hull Number: this is found on the cockpit wall near the seat
- Invoice No.: is your Order ID
- Vehicle Identification No.: can not be filled out / not applicable
- Year: this is the last 2 digits of the Hull ID in YYYY
- Place of Construction: is China unless otherwise noted on the product page
- Shipping Weight: is the assembled boat weight
- HP, GVWR & No. Cyls.: can not be filled out / not applicable 
- Series or Model: is your kayak model name e.g.: Rubicon, Raptor
- Length, Beam, Draft & Capacity: these can be found on the product page on our website for your model. Use the search in the header to find your model and the page.
Step 3: Save the form and email it to us to sign at info@reelyaks.com
Step 4: We will sign it and send you a digital copy.
Step 5: If you need a printed copy then go here.
- Choose 'double sided copy' and 'Laser (60 lb.)'.
- Upload the file we have signed.
- Proceed to checkout and pay $9.99 for the delivery.
